Based in Chillicothe, Ohio with offices in Washington Courthouse and Waverly, and currently serving individuals in Ross, Pike, Fayette, and Scioto counties; Frontier Community Services is committed to providing quality residential-based services to individuals with intellectual and/or developmental disabilities.

 

Through Supported Living and Home and Community based Waivers, Frontier Community Services offers adults with intellectual and/or developmental disabilities the support necessary so that they may live, work, and otherwise participate in their community. 

 

Frontier Community Services provides a variety of support services based on the individual needs of each person and determined by the Individual Service Plan (ISP). Supports may include:

   

 

 

 

  • Basic personal care and grooming, including bathing, care of the hair and assistance with clothing;
  • Assistance with helping the individual to and from the bathroom or similar needs;
  • Assisting the individual with self-medication or provision of medication administration for prescribed medications, and assisting the individual with, or performing health care activities
  • Performing household services essential to the individual's healthand comfort in the home such as light housekeeping.
  • Assessing, monitoring, and supervising the individual to ensure the individual's safety, health, and welfare;
  • Accessing the community for recreation, leisure, or personal errands and appointments.
  • Individual training and reinforcement in skills such as: money management, cooking and dietary needs, household tasks, accessing the community, and personal safety.

 

Services are provided in a variety of settings including:  licensed and regulated homes, apartments, leased homes, as well as living with family members. Residential Services are carried out by well-trained staff that undergo monthly continuing education. On-call staff, managers and RN’s are available 24 hours per day 7 days per week to respond in case of emergencies.

 

To be eligible for services, individuals must meet certain criteria and are evaluated by the county board of mental retardation and developmental disabilities in the county that the individuals expect to receive services.
